Differential Diagnosis of Pleural Effusions*

چکیده
Carcinoembryonic antigen (CEA), orosomucoid beta2 microglobulin, and alpha fetoprotein were quantified in the pleural fluid and serum of 58 hospitalized consecutive patients in order to differentiate malignant from nonmalignant effusions. Cytologic examination of the effusions was also performed. The orosomucoid assay was the most helpful in identifying malignant effusions; higher values (>100 mg/100 ml) were found in 74 percent (26/35) of malignant effusions. Quantitation of carcinoembryonic antigen had a high specificity (95 percent) but a low sensitivity (36 percent) for the detection of malignant pleural effusions. The parallel quantitation of CEA

Background & Aim: Adenosine deminase(ADA) can be used in the diagnosis of tuberculous pleural effusions. In countries with high prevalence of tuberculous pleural effusions, specificity and sensitivity for ADA test is high, therefore it is an integral part of a diagnostic workup of lymphocyte-rich exudative body fluids, and it is a cheap and economically cost-effective test. No study has bee...

Introduction: Cancers are the most prevalent causes of exudative pleural effusions after para-pneumonic pleural effusions. Despite the combination of the pleural fluid cytological studies and pleural biopsy, diagnosis could not be reached in an important number of cases. In an attempt to improve the value of pleural fluid analysis in the diagnosis of malignant pleural effusion, some studies...
متن کاملDiagnosis of pleural effusions.
Sixty patients of pleural effusion with different aetiology are described. Various microbiological and biochemical parameters were done simultaneously in blood and pleural fluid to differentiate tuberculosis and non-tuberculosis effusions. Some biochemical tests were thought to be helpful in differential diagnosis but no single parameter was found diagnostic. Routine investigations of pleural f...
متن کاملExpression of cytokeratin MNF116 and vimentin in pleural serous effusions.
The purpose of this study is to evaluate the value of cytokeratin (CK) MNF116 and vimentin in the differential diagnosis of malignant pleural effusions. There were evaluated smears from 30 patients with pleural effusions stained with May-Grünwald Giemsa and Papanicolaou techniques for the routine cytological diagnosis. Additional smears were immunostained with CK MNF116 and vimentin using LSAB2...
متن کاملDiffusion-weighted MRI in the characterization of pleural effusions.
PURPOSE To evaluate the value of diffusion-weighted imaging (DWI) in the differential diagnosis of pleural effusions. MATERIALS AND METHODS 58 pleural effusions (21 transudative, 37 exudative) were included in this prospective study. Single-shot echo-planar spin echo DWI was performed with two b factors (500 and 1000 s/mm (2)), and apparent diffusion coefficients (ADCs) were calculated. On di...
متن کاملEfficacy and safety of diagnostic thoracoscopy in undiagnosed pleural effusions.
BACKGROUND The differential diagnosis of pleural effusions can present a considerable challenge, and the etiology of pleural effusions varies depending on the population studied. OBJECTIVE This study aimed to assess the efficacy and safety of medical thoracoscopy in the diagnosis of patients with undiagnosed pleural effusions in a Chinese population. METHODS Between July 2005 and June 2014,...
